Each individual's desires are explored through collaboration with the individual, support professionals, family members, and friends. The creative team works together to meet these challenges and find ways to help the person achieve his or her goals, hopes and dreams. Services and supports are identified and actualized through a Person-Centered Plan. This plan assists individuals in creating an enhanced lifestyle which includes community integration, encourages healthy relationships, develops self-esteem and self-sufficiency, and establishes criteria to determine support satisfaction.
